Hairstyles by Patricia operates professional full-service salons in assisted living centers and retirement homes throughout the metro Phoenix area.  We employ only professional Arizona-licensed cosmetologists.  In the unlikely event of an accident, our staff is fully insured.

OUR RESPONSIBLE PARTY FORM

Each of our clients must have a Responsible Party Form for services to be performed.  The form tells us what services are authorized to be performed and the time interval in which to perform them.  Also, since many of our clients are not capable of paying for services or do not have the access to funds to do so, it is a record of who is responsible for payment.  You may download a copy of the form by clicking here.  Simply print it out, fill it in, and mail it to the address on the form.

Below you will find a sample price list from one of our facilities.  Prices vary from location to location, we provide this information only as a guide.  You may contact the salon at the individual facility (phone numbers can be found here) or you may contact our office here and we can email you a complete list.
